How to Access your VA Records
Dr. Miller requires documentation of your current VA rated disabilities, as well as any records of treatment you receive at the VA. Below is information on how to access those records.
Uploading a list of your current VA disabilities:
To find your current VA-rated conditions, log in to VA.gov, select the "Disability" section, and use the "View Your Disability Ratings" tool, or check your decision letters for details on service-connected conditions and percentages, using your DS Logon, ID.me, or My HealtheVet account for access, which shows finalized ratings and statuses for claims in process.
Start by logging into VA.gov, select Disability from the menu on the left as shown below.

Then, click "View your VA disability rating"

Your VA disability ratings page should look something like this. Take a screenshot of this page on your computer or phone, and then upload the screenshot to the folder Dr. Miller provided. If you don't know how to take a screenshot, you can also just type out the conditions or copy/paste them into a Word document that you can then upload to Dr. Miller:
